DECIDING ON SDP
- Self-determination is another model that is available to individuals and families upon their request. Arranging self-determination is a multi-step process that includes several factors. It's important that individuals and families are informed when deciding whether to choose self-determination. In this brief presentation, the factors to consider before enrolling in self determination are discussed.
TRANSITION & ENROLLMENT
- Transitioning into Self Determination is a process that takes time and requires working closely with Regional Center. It is a person-centered process that requires creating goals for an IPP, developing a budget and spending plan; meeting and working with the FMS agency. In these presentations, things to consider when transitioning into self determination will be discussed.
PARTICIPATION / RESOURCES
- Person-centered living is an ongoing process, and individuals and families have new responsibilities to manage the plan, working with staff, working with facilitators, and finding services. In this presentation, tips on how to be successful in the Self-Determination Program will be shared.
IPP PLANNING PROCESS
- Being in self-determination is a unique experience. It is new and everyone continues to live a more self-determined life. There are dozens of resources available to individuals and families. In this presentation a review of staff, facilitators, resources, advocate groups, and parent networks will be shared with suggestions on who and how to receive supports when needed.
